If the allegations are borne out, then the principal of a Queens middle school should be fired, declared ineligible for future employment, stripped of pension if a way can be found, and the District Attorney should carry on its own investigation separate and apart from any charade of a Department of Education inquiry.

And if the evidence leads to it, the principal should be suitably charged with an offense that might have to be upgraded to criminally negligent homicide.

Why?

An assistant principal suffered cardiac arrest in school. A defibrillator was on site and a teacher trained in its use nearby. Without immediate intervention within a few minutes, death or severe permanent disability were likely.

But there was no such timely action. There was no “Code Blue” to signify such an emergency. Not because there was no operational public address system. Not because of miscommunication.  But because there was no communication. It was a “perfect storm” of snafus enabled by human error.
