NASA doesn't have anything to do with the rocket.  They simply pay for its use.  Orbital Science builds their own rockets.

That's not exactly a tribute to either institution.

Excluding the man rated rockets, most modern US rockets have not been built by NASA.

The Delta II, III, and IV are build by Boeing (formerly by McDonnell Douglas) and the Atlas V is build by Lockheed which now work together as United Launch Alliance.

Orbital Sciences not only flies the Antares rocket, but also the Minotaur (built from decommissioned ICBMs) and the air launched Pegasus rocket.  They also build parts of the US missile defense system.

SpaceX flies the Falcon 9 (and its different versions) for both NASA and private companies.

Rockets fail.  They are extremely complex systems which have zero tolerance for failure.  One little thing breaks or malfunctions and the result is a big boom.

None of these rockets are "man rated," meaning the standards are not nearly as high for reliability because no lives are at risk if one fails.

That is what I get of missing too many national meetings. I know a guy who is a Lt Colonel from that base.

Thanks for the graphic on the primary and secondary targets of a nuclear attack.  I wish it was a little bigger, I tried to blow it up larger and it got fuzzy, but it looks like there is a black blob right over where I live.

The map is not old. The targets still are the same. So if you live in a black and purple area you may want to heed warnings of evacuation.The British have every attack scenario plotted out. They even how the leadership is broken down to the city controller, food stocks, fuel stocks, army and police.

I used to live about ten miles from the launch pads (as the crow flies).

I saw a lot of rockets blow up on launch. I was on the sand in Cocoa Beach when the Challenger blew up.

There are various ways of measuring the danger of spaceflight based on comparing the number of fatalities to the number of non-fatal spaceflights.

About two percent of the manned launch/reentry attempts have killed their crew, with Soyuz and the Shuttle having almost the same death percentage rates. Except for the X-15 (which is a suborbital rocket plane), other launchers have not launched sufficiently often for reasonable safety comparisons to be made.

About five percent of people who have been launched have died doing so. As of November 2004, 439 individuals have flown on spaceflights: Russia/Soviet Union (96), USA (277), others (66).[citation needed] Nineteen have died while in flight: one on Soyuz 1, one on X-15-3, three on Soyuz 11, seven on Challenger, and seven on Columbia. By space program, 16 NASA astronauts (5.8%) and four Soviet cosmonauts (4.1% of all the people launched) died while in a spacecraft.

Soyuz accidents have claimed the lives of four cosmonauts. No deaths have occurred on Soyuz missions since 1971, and none with the current design of the Soyuz. Including the early Soyuz design, the average deaths per launched crew member on Soyuz are currently under two percent. However, there have also been several serious injuries, and some other incidents in which crews nearly died.
