Returning from a casino trip, dozens of passengers slept as their tour bus sped down a California highway before dawn. Moments later, it slammed into a stopped semi-trailer with devastating force. Thirteen people lost their lives and dozens more were injured. Investigators uncovered fatigue, poor health, safety violations, and multiple failures that contributed to the crash.
