The initiative, the largest federal investment in rural health care in U.S. history, was created through President Trump’s Working Families Tax Cuts legislation, a Republican-led effort that every Democrat in Congress opposed.
“More than 60 million Americans living in rural areas have the right to equal access to quality care,” Health and Human Services Secretary Robert F. Kennedy Jr. said in a statement. “This historic investment puts local hospitals, clinics, and health workers in control of their communities’ healthcare. Thanks to President Trump’s leadership, rural Americans will now have affordable healthcare close to home, free from bureaucratic obstacles.”
The Centers for Medicare & Medicaid Services, led by Administrator Dr. Mehmet Oz, awarded the fiscal year 2026 allocations totaling $10 billion, with the full $50 billion to be distributed over five years through 2030.
Funds will support strengthening rural health workforces, upgrading facilities and technology, and implementing innovative care models to improve access for the nation’s heartland communities.
“Today marks an extraordinary milestone for rural health in America,” Oz said. “Thanks to Congress establishing this investment and President Trump for his leadership, states are stepping forward with bold, creative plans to expand rural access, strengthen their workforces, modernize care, and support the communities that keep our nation running.”
Awards for 2026 range from $147 million in New Jersey to $281 million in Texas, with allocations based on factors including rural population, infrastructure needs, and states’ policy commitments to efficient, patient-centered reforms.
