In 1980's, the Iranians successfully used North Korean WWI-style underwater mines in Persian Gulf.
In 1988, USS Samuel B. Roberts (FFG-58) struck one of those Iranian mines.
US Navy was unprepared. They had not prioritized mine sweeping in the Persian Gulf. They were blinded by technical progress to consider that an enemy would still use such crude weapons.
Much like US fighter planes early in the Vietnam War that only had missiles, not guns or cannons like the enemy did.
