No, no, no, and no.  Trump remains President at least through Jan 20 of next year.  If Trump dies, then Pence becomes President and selects a new VP.  If Pence then dies, the new VP becomes President, and he/she picks a new VP.

Trump's physician stated that Trump will continue with his Presidential duties. Trump is not incapacitated, so VP Pence has not taken on Presidential duties under the 25th Amendment.
