She isn't fat.  One day she was slightly obese, wearing normal clothes.  Then, she was in hospital, and from then on she was/is wearing something around her body - her clothes changed in one day, to tent like dresses to cover her with pants underneath.  I believe she is in a body brace of some type. 

She is normal size in her face, her neck, her arms, and in her upper body until just under her breasts which is where the tent starts. 

The purse is a smallish box; could not be comfortably worn or handled.  Would never buy a purse like that even if it was $2.98 or less, or $100,000 or more.
