US Navy warships involved in fatal collisions in the Pacific earlier this year were both to blame, new report finds

Two fatal crashes involving U.S. Navy warships and commercial vessels were caused by preventable errors by the sailors on board the ships, a new report has found.

The accidents, in which 17 sailors were killed, raised questions about Navy training and the pace of operations, prompted a Congressional hearing and the removal of a number of officers.

Speaking as the report was released, Chief of Naval Operations Admiral John Richardson said 'we must do better'.

'Both of these accidents were preventable and the respective investigations found multiple failures by watch standers that contributed to the incidents,' he said.
