One person has died and another six were injured when a ride malfunctioned at the Ohio State Fair, according to CBS affiliate WBNS-TV, citing the Columbus Division of Fire.

The station adds that five of the injured are listed in critical condition. The last was in stable condition.

Officials with the Ohio Department of Agriculture identified the ride as the "Fireball."

Wednesday evening Ohio Gov. John Kasich released a statement on Twitter saying that he is "terribly saddened by this accdent, by this loss of life and that people were injured."

The ride allegedly was rigorously inspected before the tragedy.   If true, the thought of sabotage crossed my mind.  But if so, why?

From the video, it could have easily killed all 4.  It detached near the ground.  If it had held a second or two longer, it would have come off near the top of the swing.

I saw that the one who was killed on impact was 18 years old.  Have they released the ages of the others who were injured?

From the video of the accident, it looks like one of the rows of seats that was held together at the spindle arm broke loose as the pendulum swing was nearest the ground and the opposite side of seats plowed into the just-detached row, which broke loose the restraints of the 4 seats on the opposite side and flung two people out of their seats as the arm swung up.  The detached row of seats flipped upside down and crashed onto the pavement with one person still strapped into their seat.  He landed headfirst and looks like his legs were broken in multiple places.  Not sure if that was the fella who died or not.

Deadly accident at Ohio State Fair caused by corrosion, says ride maker

The manufacturer of an amusement park ride that malfunctioned at the Ohio State Fair in July said on Sunday that the deadly incident was caused by "excessive corrosion."

The Fire Ball ride collapsed on July 26, killing one person and injuring seven others. The ride consists of at least six rows of seats that spin around 40 feet above the ground as the entire structure moves like a pendulum.

A video posted online appears to show several people on the Fire Ball seconds before parts of the structure, including a full row of seats, split up and fell. The ride broke apart while in motion.

Rhonda Burgess said one of her sons was standing in line at a nearby ride when the incident took place.
"The ride had four riders per cart. This piece snapped off and the riders came out of the cart," she said. "At least two (people) flew through the air at least 20 feet before landing on their backs on the concrete."

Albert Kroon, product manager for the Dutch manufacturer, said in a statement that an investigation at the scene in Columbus determined that "excessive corrosion on the interior of the gondola support beam dangerously reduced the beam's wall thickness over the years." The ride was 18 years old...

Corrosion on the interior of a support beam wasn't likely caused by being taken apart and put together at other joints.  However, I can see how being exposed to Ohio road salt could be a contributing factor.
