Folks should stop pretending that HIV/AIDS patients are anything other than permanent wards of the State, wholly dependent on government-paid welfare for their continued survival.  As such, their care, to the extent that "we" want to subsidize it, should simply be paid for directly out of welfare - state-sponsored charity - funds.  HIV/AIDS is not the sort of "opportunity" that slackers and parasites would arbitrage in order to take unfair advantage of government largesse, so there is no "tragedy of the commons" risk to directly funding charity for them through government welfare funds.
